The Essential is an individual health plan available between ages 18 and 64. Newborn babies may be added as a dependent after 14 days and coverage continues to age 25. The plan covers:

• Prescription drugs

• Maternity

• Diagnostic services

• Hospitalization

• Surgery

• Tubal ligation

• Radio and Chemotherapy

• Renal dialysis

• Local ambulance

• HPV Vaccine

• Hearing Aid

• Immunization to age 12

• Overseas non-emergency

• Overseas emergency

• Dental and Optical

It also carries a Major Medical Lifetime Benefit of $4,000,000.  Coordination of benefits with other health plans is available. A previously enrolled member of a guardian group health plan may convert to this individual plan within 30 days of termination from the group plan.
